Job Summary We're currently supporting Cheshire and Merseyside ICB in recruiting an All Age Continuing Care (AACC) Broker Administrator to join the Sefton and Liverpool AACC Team. This is an excellent opportunity for an experienced administrator, broker, care coordinator or commissioning professional to play a vital role in sourcing and coordinating care packages for individuals with complex health needs. You'll work closely with healthcare professionals, care providers, local authorities and families to ensure timely and cost-effective care arrangements are in place. The role offers the chance to make a real difference to patient outcomes by supporting hospital discharges, Continuing Healthcare services and ongoing care provision across the community. Who We're Looking For You're a highly organised professional with experience coordinating services within health and social care. You enjoy building relationships, managing competing priorities and finding effective solutions to complex situations. You'll be confident communicating with a range of stakeholders and able to work independently while maintaining accuracy and attention to detail in a fast-paced environment. Key Responsibilities
